DFO Training Agreement, Policies & Liability Waiver
Please read this agreement carefully. By checking the acceptance box when you register, you (and, for participants under 18, the parent or legal guardian) agree to all of the terms below.
Booking Policy
- All sessions must be booked at least 24 hours prior to the requested start time.
- Session times are not guaranteed until confirmed by DFO Training.
Cancellation Policy
- An 8-hour cancellation notice is required for all sessions.
- Cancellations made within 8 hours of the scheduled session will result in the loss of that session, and no credit will be issued.
- Weather-related cancellations and coach emergencies do not count toward the cancellation policy and will be rescheduled accordingly.
Fees & Refunds
- All fees are non-refundable except where a refund is required by law.
- Session packages are for session credit only, expire according to the terms shown at the time of booking, and unused sessions are not redeemable for cash.
Assumption of Risk & Liability Waiver
I understand that participation in soccer training, fitness activities, conditioning, speed and agility work, and related activities involves inherent risks including injury, illness, disability, or death. I voluntarily assume all risks associated with participation in DFO Training activities.
I release and hold harmless DFO Training, its owners, coaches, staff, affiliates, and representatives from any and all claims, liabilities, damages, costs, or expenses arising from participation in any DFO Training activity, including claims based on the ordinary negligence of the released parties, to the fullest extent permitted by law.
Indemnification
I agree to indemnify and hold harmless DFO Training and its owners, coaches, staff, affiliates, and representatives from any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising out of or related to my or the participant's involvement in DFO Training activities.
Medical Authorization
In the event of an emergency, I authorize DFO Training staff to seek medical treatment for the participant if a parent or guardian cannot be reached immediately. I confirm that the participant is in good health and physically able to participate, and I accept responsibility for any medical costs incurred.
Photo & Video Consent
I grant DFO Training permission to photograph, record, and film the participant during training sessions, camps, clinics, and events. I understand these images and videos may be used on DFO Training social media accounts, websites, promotional materials, advertisements, flyers, marketing campaigns, and other DFO Training platforms without compensation.
Authority to Bind Minor
If the participant is under 18 years of age, I represent that I am the parent or legal guardian with full authority to accept this agreement on the participant's behalf and to waive and release claims on behalf of the minor.
Governing Law & Severability
This agreement is governed by the laws of the State of Texas. If any provision of this agreement is found to be unenforceable, the remaining provisions will continue in full force and effect.
Acknowledgement & Electronic Acceptance
By checking the acceptance box, I acknowledge that I have read, understand, and agree to all policies, terms, and conditions contained in this agreement, and I agree that my electronic acceptance is the legal equivalent of my handwritten signature.
